Temporary Assistance Case Manager for Elderly Care in Kungsör

The Investigation Unit for Assistance is divided into two teams. One team handles cases within elderly care, and the other team handles cases within disability support and transport services. We also have contact persons, support families, and foster homes according to the LSS (Act for Support and Service for Persons with Certain Functional Impairments). Job Description

As a temporary assistance case manager at Kungsör Municipality, you work with authority exercises primarily concerning the elderly. You are responsible for investigating, assessing, deciding on, and following up on measures according to the Social Services Act. You adhere to the laws, regulations, agreements, and guidelines that regulate the area of operation. Depending on your experience, case handling may also involve individuals with disabilities, social psychiatry, as well as local and national transport services.

The duties also include collaboration with the individual, relatives, colleagues, service providers, as well as other professions and authorities.

Qualifications

We are looking for you who have a degree in social work or another equivalent academic education where you have studied social law. We prefer that you have experience with authority exercises within any of the areas of elderly care, LSS, or social psychiatry. Experience working in the operational systems Lifecare, Cosmic Link, and the investigation method IBIC is an advantage. Since travel is part of the job, you must have a valid B-driver's license.

You have a genuine desire to make a difference in people's lives. You have good collaboration skills, are structured, and actively contribute to creating job satisfaction and well-being in the work team. Your approach is trustworthy and respectful.
